NCQA Disease Management Accreditation Seal

NCQA accreditation

Health Dialog was awarded full accreditation by the National Committee for Quality Assurance (NCQA) in Patient Oriented Disease Management for the following conditions:

  • Coronary heart disease (CHD)
  • Diabetes
  • Congestive heart failure (HF)
  • Asthma
  • Chronic obstructive pulmonary disease (COPD)

Full accreditation is awarded for three years to organizations that meet or exceed NCQA’s rigorous standards.
