Health Dialog’s Mission is to Improve Population Health and Reduce Costs
Health Dialog’s philosophy is simple: We provide truly personalized population health management support services that address the unique needs of every individual. All of our population health management programs are designed to deliver the right intervention to the right patient at the right time.
Why Choose Health Dialog?
Our people are the strength of our program.
Through over 20 years of experience, we know that no form of health support is more effective or meaningful to people than a direct, person-to-person interaction with a trained professional. That's why we staff our population health management programs with highly-trained, experienced Health Coaches.
Our registered nurses are licensed in all 50 states and the District of Columbia, and average 13 years of experience. They use their clinical knowledge plus a variety of tools and resources to support the needs of all call types and populations in order to evaluate and then guide callers to the most appropriate level of care.
We target our programs to drive the greatest impact.
Health Dialog’s powerful analytics and predictive models were developed with one goal—to identify actionable health management opportunities that improve people’s lives and reduce the cost of healthcare.
Unlike traditional methodologies, our analytics combine raw healthcare data—including claims, health risk assessments, biometric screenings, and EMR data—with unique assets, such as census data (to understand geographic, cultural, language, and socioeconomic factors), financial data (to assess purchasing decisions), and the Dartmouth Atlas (to predict the likely use of medical resources and services based on geographic location).
Our population health track record speaks for itself.
Our partnerships have achieved some remarkable results, from reducing hospitalizations, increasing medication adherence, and delivering significant savings. Just a few of our measurable results include:
- 66% of callers with pre-intent to visit the ER or call 911 were appropriately redirected to a less emergent level of care
- 12.5% fewer hospital admissions
- Reduced medical and pharmacy costs by $7.96 PMPM
